The Alloy Swivel hook used on a Vestil Boom was not equipped with a safety latch, operating without the safety chain on or about 6/11/2021.The driver of the delivery truck lost a leg after being crushed by falling slabs that were knocked over as the truck bed moved during the unloading operation utilizing a Komatsu Forklift.
