29 CFR  1910.28(b)(15):29 CFR 1910.28(b)(15): Walking-working surfaces not otherwise addressed. Except as provided elsewhere in this section or by other subparts of this part, the employer did not ensure each employee on a walkingworking surface 4 feet (1.2 m) or more above a lower level is protected from falling by one of the systems listed in 1910.28(b)(15)(i)-(iii).  Warehouse, Aisle 114, 795 Jubilee Drive, Peabody, MA  a) On or about 6/21/21, an employee operating an order-picker powered industrial truck was not protected from falling by means of guardrails or a personal fall arrest system. An employee was not wearing a personal fall arrest harness or belt, and fell from the powered industrial truck to the floor below.
