OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard:   a. Warehouse: On or about June 22, 2021, and at times prior thereto, employees were exposed to a struck-by hazard while operating powered industrial trucks in areas where rack systems, stocked with palletized products, were unanchored.  b. Warehouse: On or about June 22, 2021, and at times prior thereto, employees were exposed to a struck-by hazard while operating powered industrial trucks in areas where rack systems, stocked with palletized products, did not have a displayed a plaque with the maximum permissible unit load.

29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:  On or about June  22, 2021, and at times thereto, employees were exposed to a slipping hazard while working in a warehouse floor that had multiple water leaks.

29 CFR  1910.178(l)(1)(ii):The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck.  On or about June 22, 2021 employees were exposed to a struck-by hazard when operating forklifts by an operator who was not trained or evaluated by the employer.
