29 CFR  1910.178(l)(3)(ii)(E):  Training in work related topics did not cover narrow aisles and other restricted places where the vehicle will be operated;  (a)  The employer did not develop and train employees on a detailed plan for the use of the Crown guided wire system that was installed for use in narrow aisles in the warehouse, which included the direction of travel, when and where a PIT was allowed to get on the wire, what circumstances would allow a PIT driver to come off of the wire,  whether or not employees were allowed to drive in reverse on the wire, or how often a PIT should have the calibration of the wire system checked.

29 CFR  1910.178(l)(4)(ii)(B):  Refresher training in relevant topics shall be provided to the operator when the operator has been involved in an accident or near-miss incident.    On or about June 24, 2021, it was found that the employer did not provide  refresher training for each powered industrial truck operator who was involved in the following accidents or near miss incidents:  (a)  On 6/21/2021, a fork lift operator came off the guided wire system to let another operator pass.  An operator forgot to reattach to the wire system and hit a rack, causing the A level shelf to fall off the bracket.  No refresher training was conducted.  (b)  On 6/10/2021, a restocker operating vehicle SP52, was making a turn into an aisle in a congested area when they got too close to a rack and scraped the orange guard.  No refresher training was conducted.  (c)  On 5/26/2021,  an IC restocker operating vehicle SP06, hit a wall while making a U turn from a parked position, leaving a dent in the wall.  No refresher training was conducted.  (d)  On 5/10/2021, an IC restocker operating vehicle RR98, got the right fork into a pallet and the left fork hit the pallet.  The pallet broke in half and the boxes fell off.  No refresher training was conducted.  (e)  On 5/4/2021, an IC restocker operating vehicle SP80, when the vehicle jumped the wire while going full throttle and veered left, striking an upright and a pallet.  No refresher training was conducted.    (f)  On 4/29/2021, an inventory control lead operating vehicle RR20, struck a beam with the mast of the vehicle.  No refresher training was conducted.    (g)  On 4/20/2021, a forklift operator operating vehicle RR98, was putting a pallet away and did not make sure the mast was all the way down before exiting the cage.  The vehicle mast struck the door sensor, knocking it off the bracket.  No refresher training was conducted.
