29 CFR  1910.28(b)(3)(i): The employer did not ensure each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following: covers, guardrail systems, travel restraint systems, or personal fall arrest systems, as required by sub-items A through D of this section.  On or about June 28, 2021, and at times prior thereto, where employees working near unprotected skylights were not protected against falling through the skylights.  This condition exposed an employee to fall hazards of approximately 35 feet.

On or about June 28, 2021, and at times prior thereto, where an employee was not provided the training, knowledge and skills necessary to utilize fall protection and to recognize and minimize the exposure to fall hazards.

29 CFR  1910.132(d)(1):The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment.  On or about June 28, 2021, and at times prior thereto, the employer did not assess the job site to determine the hazards present and the personal protective equipment required to protect the employees from those hazards where an employee was not provided fall protection exposing the employee to fall hazards.
