29 CFR 1926.300(b)(4)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or about April 2, 2021, the point of operation of a Dewalt table saw did not have guards in place and an employee sustained an amputation to two fingers.

29 CFR 1904.39(a)(2):Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. Located at Limited Contracting in Pickerington, OH: The employer did not report an employee amputation within 24 hours of the knowledge of the amputation. On or about April 4, 2021 an employee sustained an amputation to two fingers while operating a table saw.
