29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: At the jobsite located at 17260 W. 169th Terrace, Olathe, Kansas employees conducting roofing operations were exposed to fall hazards in that fall protective systems was not being used from a 8/12 pitch roof, approximately 23 feet from the roof eave to the ground below.

29 CFR 1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the jobsite located at 17260 W. 169th Terrace, Olathe, Kansas employees engaged in roofing activities were exposed to fall hazards in the instances listed below, in that the two aluminum extension ladders had side rails not extended at least 3 feet above the upper landing surface. 1. The aluminum Werner 20 foot extension ladder located at the eave of the roof, and approximately 13 feet and 2 inches to the rock ground 2. The aluminum 18 foot extension ladder located above the main entrance door between the two upper level peaks, and approximately 6 feet from the upper level roof eave to the lower level roof peak pitched at 6/12.
