On or about June 27, 2021 at the jobsite located at 3861 S 9th St, Salina, KS 67401 employees engaged in welding activities were exposed to hazards of falling through holes while working from heights in that employer failed to ensure the opening hole was properly covered to prevent accidental falling. After removing a piece of machinery, a hole was exposed in the metal flooring. The hole was not properly covered leading to an employee falling through the hole and to the basement floor approximately 33 feet. The employee received severe injuries from the fall.
