29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about June 27, 2021 at the jobsite located at 3861 S 9th St, Salina, KS 67401 employees engaged in welding activities were exposed to hazards of falling through holes while working from heights in that employer failed to ensure the opening hole was properly covered to prevent accidental falling. After removing a piece of machinery, a hole was exposed in the metal flooring. The hole was not properly covered leading to an employee falling through the hole and to the basement floor approximately 33 feet. The employee received severe injuries from the fall.

An employee tripped/stepped back into this hole and received serious injuries to their head and back.

29 CFR 1926.501(b)(4)(iii):Each employee on a walking/working surface was not protected from objects falling through holes (including skylights) by covers: On or about June 27, 2021 at the jobsite located at 3861 S 9th St, Salina, KS 67401 Employees were removing furnace equipment from the top level of the catwalk and left holes in the floor where the troughs used to be. 2 Employees were exposed to falling debris through holes. The employees were using these holes to drop debris down to the floor. An employee could've been struck by a piece of debris causing serious injury or death.

29 CFR 1926.850(h): Debris was dropped through holes in the floor without the use of chutes into an area not properly barricaded: On or about June 27, 2021 at the jobsite located at 3861 S 9th St, Salina, KS 67401 employees from Hosea Project Movers were contracted to perform demolition duties for Signify Phillips Lightbulb Co. 2 Employees were using the exposed holes in the upper landing floor to drop debris down onto the floor. There was no barricade to keep employees from entering the area where debris was being disposed. Employees could walk beneath this area when going towards the exit door.
