The platform was 144 inches wide and the width of the telehandler was 102 inches without the outriggers extended.  Two employees were exposed to a fall of approximately 30 feet when using the job made platform without guardrails on all sides or any other form of fall protection while collecting tools and debris from a third floor window opening on the east end of the building.

Employees were working from elevated aerial lifts without fall protection while framing the exterior walls of the fourth floor, thereby exposed to fall hazards of up to 40 feet.  Roberto Reyes Construction, LLC was previously cited for a violation of this Occupational Safety and Health standard, which was contained in OSHA inspection number 1483740, citation number 1, item number 2, and was affirmed as a final order on 08/21/2020, with respect to a workplace located at 5285 High Point Lakes Drive, Building 3, Westerville, Ohio 43081.
