29 CFR  1910.67(c)(2)(v): A personal fall arrest of travel restraint system that meet the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:   On or about July 28, 2021, and at times prior thereto, an employee was exposed to a fall hazard while removing banners without using a lanyard to attach to the harness worn by the employee.

29 CFR  1910.67(c)(2)(viii): An aerial lift truck may not be moved when the boom is elevated in a working position with men in the basket, except for equipment which is specifically designed for this type of operation in accordance with the provisions of paragraphs (b)(1) and (b)(2) of this section:  On or about July 28, 2021, and at times prior thereto, an employee was exposed to a fall hazard while the truck was traveling to the next work area with the basket in the elevated position and the worker not connected to the basket.
