29 CFR  1926.900(h):The employer did not take precautions to prevent potential bodily injury to employees in the loading, delaying, initiation, and confinement of each blast with mats or other methods so as to control the throw of fragments when blasting was done in congested areas or in proximity to a structure, railway, or highway, or any other installation that may be damaged.  Oak Hill Road, Standish Maine - On or about July 29, 2021, blasting mats were not used during blasting operations.
