29 CFR  1926.451(b)(8): At all points of a scaffold where the platform changes direction, such as turning a corner, the platform that rests on a bearer at an angle other than a right angle was not laid first, and platforms which rest at right angles over.  17242 Autry Pond: On or about August 19, 2021, and at times prior thereto, employees were exposed to a fall hazard to the ground below while working from a scaffold that had a platform that was not resting on bearers when changing directions of the scaffold to create a corner.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  17242 Autry Pond: On or about August 19, 2021, and at times prior thereto, employees were exposed to a fall hazard when accessing a scaffold using the cross braces instead of a ladder.  Trevino Masonry, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 1245418, citation number 1, item number 2- b  and was affirmed as a final order on 08/31/2017, with respect to a workplace located at 2814 SW Military Drive, San Antonio Texas, 78224.
