29 CFR  1926.21(b)(2):The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  A.) On or about September 7, 2021 employees were not instructed in the recognition and avoidance of hazards in that during the construction of a cardboard-waste conveyor system an employee was injured while wearing loose fitting clothing while working near the in running nip point of  the conveyor belt.

29 CFR  1926.307(e)(2)(i):Overhead horizontal belts, with lower parts 7 feet (2.128 m) or less from the floor or platform, shall be guarded on sides and bottom in accordance with paragraph (o)(3) of this section.  A.) On or about September 7, 2021 employees were exposed to in-running nip points of a conveyor under construction in that an employee was injured when their hand was caught in the in-running nip point of the conveyor.
