29 CFR  1910.134(e)(2)(i): The employer did not identify a physician or other licensed health care professional (PLHCP) to perform medical evaluations using a medical questionnaire or an initial medical examination that obtained the same information as the medical questionnaire:  a) Trans-Med Ambulance:  Employees were provided with respirator medical evaluations using the medical questionnaire, and the employer did not utilize a PLHCP that was operating within the scope of their license when performing these respirator medical evaluations, on or about September 14, 2021. 29 CFR  1910.134(e)(4)(i): The medical questionnaire and examinations were not administered confidentially during the employee's normal working hours or at a time and place convenient to the employee:  a) Trans-Med Ambulance:  Employees were provided with respirator medical evaluations using the medical questionnaire, and these questionnaires were not administered confidentially.  The employer evaluated and maintained these confidential medical records, on or about September 14, 2021.  ABATEMENT CERTIFICATION REQUIRED
