29 CFR  1910.23(b)(8): The employer must ensure that ladders are used only for the purposes for which they were designed.  a) On or about September 13, 2021, at the worksite located at 2511 Neva Road, Antigo, WI 54409, the employer did not ensure that a platform step ladder (Werner PT7406-4C) affixed with spring loaded casters (Werner 40-4HD) was used as intended, thereby exposing an employee to fall hazards. An employee stepped off of the platform step ladder to grab product off of a pallet on a racking system, which caused the step ladder to slide out from under the employee when they tried to egress back onto the step ladder.   b) On or about August 21, 2021, at the worksite located at 2511 Neva Road, Antigo, WI 54409, the employer did not ensure that a step ladder was not used for lateral and vertical access to an elevated work surface, thereby exposing an employee to fall hazards.
