29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) On or about September 24, 2021, two workers were exposed to fall and ejection hazards while elevated approximately 10 feet in a JLG 430AJ aerial lift performing paint preparation work.  b) On or about September 24, 2021, two workers were exposed to fall and ejection hazards while elevated approximately 10 feet in a Genie S-65 4X4 aerial lift performing paint preparation work.  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about September 24, 2021, a worker accessed a sloped roof by exiting an aerial lift platform without fall protection and was then exposed to falling from the unprotected side or edge of the roof.
