29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  Location - 11 Combination Street, Methuen, MA 01844.  On or about 10/6/2021, employees who work at heights were not provided fall hazard training.  Following the 7/1/2021 abatement due date for the violation issued to the company, the employer continued to expose employees to serious injuries and possibly death by not attending and providing fall protection training to employees.  Due to technical difficulties with a training provider, the abatement due date was amended to 9/15/2022.  Also, following the 9/15/2022 abatement due date as a requirement of number ten (10) of the Informal Settlement Agreement, the employer did not attend fall protection training.

Location: 11 Combination Street, Methuen, MA 01844  On or about 10/6/2021, while using a Porter Cable Finish Nailer nail gun, employees did not utilize eye or face protection in order to protect their eyes and face from flying particles.
