29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  (a) On or about October 21, 2021 live parts of electrical equipment were not guarded in that the cover for the pressure regulator switch on the Quincy air compressor was missing; exposing employees to 220 Volts of electrical energy through accidental contact.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions and the openings through which they entered were not effectively closed:  (a) On or about October 21, 2021 conductors were not protected from abrasions and the openings through which they entered were not effectively closed when the conduit on the Quincy air compressor was found pulled out of a pressure regulator switch; exposing employees working within the facility to an inhalation hazard in the event of an electrical fire.

29 CFR 1910.243(b)(2): Air hose and hose connections used for conducting compressed air to utilization equipment was not designed for the pressure and service to which they were subjected:  (a)	On or about October 21, 2021, an air hose with approximately 85-100 psi pressurized air located on the exterior South/East wall of the maintenance shop was found with a hose clamp on it exposing employees using it to a struck-by hazard. (b)	On or about October 21, 2021, an air hose coming out of a hose reel with approximately 85-100 psi pressurized air located on the exterior South wall between the four bay doors of the maintenance shop was found with a hose clamp on it exposing employees using it to a struck-by hazard. (c)	On or about October 21, 2021, an air hose with approximately 85-100 psi pressurized air located in the third bay on the North side of the maintenance shop was found with a hose clamp on it exposing employees using it to a struck-by hazard.  BIG D CONCRETE, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, (1910.243(B)(2), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1446559, CITATION NUMBER 01, ITEM NUMBER, 002, WAS AFFIRMED AS A FINAL ORDER ON 03/11/2020, WITH RESPECT TO A WORKPLACE LOCATED AT 10361 BICKHAM RD, DALLAS, TX 75220.
