Employees were thereby exposed to the hazards associated with unguarded ingoing nip points and rotating parts during the operating cycles.  b) On or about October 21, 2021,  in the production area, the employer failed to provide effective guarding to protect employees working at or near the Jumbo Straw  Extruding Machines #2. The guarding device for area between crimping wheel and straw guide with ingoing rollers, the area underneath the takeaway conveyor with exposed moving conveyor belts, and the area underneath the handwheel with ingoing rollers did not prevent employees from operating the machine while in the zone(s) of danger during the extruding process. 29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) On or about October 16, 2021, employees were exposed to machine hazards associated with moving parts when clearing straws from underneath the Take Away Conveyor on the Jumbo Straw Line #5. The employer failed to implement energy control application steps as the machine was not shut down or turn off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.  Best Diamond Plastics LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 29 1910.147(d), which was contained in OSHA inspection number 1455363, citation number 1, item number 1b and was affirmed as a final order on May 13, 2020, with respect to a workplace located at 1401 E. 98th St. Chicago, IL 60628.
