29 CFR  1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition.   On or about October 15, 2021, an employee was performing maintenance on a construction vehicle located on the side of a dirt path where heavy equipment was operated without wearing high visibility garments, exposing employees to struck-by hazards.
