29 CFR  1910.178(n)(6):  Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel:   (a)  Facility:   On or about November 11, 2021, an operator of a Crown powered industrial truck was traveling backwards and he did not keep a clear view of the path of travel of the Crown powered industrial truck.  The Crown powered industrial truck struck an employee causing severe leg injuries.

Employees work with hazardous chemicals such as, but not limited to, Technomelt EM 155 A (glue), RELEASE (cleaner and degreaser)  and Silver Grade Anti-Seize Lubricant.
