OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards from falling loads:  Site- On or about November 10, 2021 a crane with a Campbell Model E, friction-type pressure gripping lifter (clamp) attachment was being used to lift sheets of 3/8 inch steel.  The clamp was worn and was lifting multiple plates at once.  This caused the potential for crushing hazard from falling plates if the clamp was unable to support the load.

29 CFR  1910.135(a)(1):The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  Site- Employees were not protected from head injuries where the employer did not require the wearing of head protection while operating the overhead bridge crane. On or about November 10, 2021 three steel plates slipped out of a Campbell Model E, friction-type pressure lifter, while being lifted by the bay 3 Shawbox bridge crane.
