The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  On or about November 19, 2021, at an establishment located at 4617 South Taylor Drive in Sheboygan, Wisconsin, in the Paint Flake Department; an employee was exposed to machine hazards associated with rotating rollers and a conveyor belt while attempting to remove debris from the line, on the Paint Flake Line B-2. The employer did not ensure that employees implemented energy control application steps prior to cleaning the Paint Flake Line. The machine was not deenergized prior to the employee engaging in the service and/or maintenance activity.   As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], LOTO device application [1910.147(d)(4)], and verification of isolation [1910.147(d)(6)], were not implemented to protect employees from machine servicing hazards.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:   a) On or about November 19, 2021, at an establishment located at 4617 South Taylor Drive in Sheboygan, Wisconsin, in the Paint Flake Department; on the 28 Paint Flake Lines, employees were exposed to in-running catch point hazards created by the main conveyor belt and rotating rollers on the front and rear of the line.  b) On or about November 19, 2021, at an establishment located at 4617 South Taylor Drive in Sheboygan, Wisconsin, in the Paint Flake Department; on the 28 Paint Flake Lines, employees were exposed to potential caught in hazards created by the unguarded rotating components of each chipper box. Each box was equipped with a hinged guard that was not affixed closed and/or interlocked to stop the machine's movement when it was opened.
