29 CFR  1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):   a.  On or about December 2, 2021, in the facility, the employer did not ensure the "Lockout /Tagout Energy Control Procedure" used by employees for service and maintenance of the Crown RR/RD 5200 Series, Serial Number 1A339366, clearly and specifically outlined the scope, purpose, authorization, rules and techniques to be utilized for the control of hazardous energy. The omission of the information including, but not limited to, the techniques for proper control of hazardous energy exposed employees to struck-by hazards.

29 CFR  1910.147(c)(6)(i): The employer did not conduct an annual inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:   a.  On or about December 2,  2021, in the facility, the employer did not conduct periodic inspections of the energy control procedures at least annually to ensure that procedures and requirements of the standard were being followed.
