29 CFR 1910.178(l)(3)(i)(K): Powered industrial truck operators shall receive initial training in refueling and/or charging and recharging of batteries. On or about December 7, 2021, the employer at the Amcor facility at 975 W. Main Street in Bellevue, Ohio did not train employees on how to properly charge the lift trucks, overheating the lift truck's battery and exposing nearby employees to the battery's gases, causing illness including but not limited to acute coughing, shortness of breath, eye, nose, and throat irritation, nausea, headache, dizziness, and upper chest pain.

29 CFR 1910.178(o)(1): The employer failed to develop workplace standards stating only stable or safely arranged loads shall be handled. Caution was not exercised when handling off-center loads which cannot be centered. On or about December 7, 2021, the employer at the Amcor facility at 975 W. Main Street in Bellevue, Ohio did not safely arrange or stabilize loads when using the pallet jacks, allowing employees to push a load on the ground while moving another load on the pallet jack, damaging the pallet jack and exposing nearby employees and drivers of other Powered Industrial Vehicles (PIV) to struck by hazards.
