29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): (a) On or about 11/24/2021, and at times prior to - 900 Goodwin Road, Guntersville, AL., in the hatchery, at the macerator, employees sanitizing and disinfecting the hatchery macerator were exposed to caught-in and struck-by hazards from macerator blades, auger screw conveyors, and other moving parts because the specific LOTO Energy Control Procedure (ECP) for the macerator entitled PDC Hatchery, - Specific LOTO Instructions, and dated 10/18/18 did not include all the required information such as, but not limited to, an explanation of its intended use, steps to isolate energy sources, steps to correctly apply lockout device(s), and steps to follow to verify energy isolation is accomplished and effective.
