29 CFR  1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  On or about December 13, 2021, employees were exposed to a fall hazard while working in an excavation without utilizing a safe means of egress.

29 CFR  1926.652(b):The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4):  On or about December 13, 2021, employees were exposed to a crushed-by hazard while working in an excavation without it being protected from cave-ins by an adequate protection system.
