29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  On or about December 16, 2021, and at times prior thereto, employees were performing maintenance on the Shear and Auto Brake machines without the employer developing procedures to isolate all energy sources, exposing the employees to the hazards of unexpected startup.

29 CFR 1910.147(c)(5)(i):Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources.  On or about December 16, 2021, and at times prior thereto, the employer did not provide lockout tagout devices to control hazardous energy when employees were performing servicing and maintenance on machines and equipment, exposing employee's to hazards including, but not limited to caught-in moving parts.

On or about December 16, 2021 and at times prior thereto, the employer did not provide training to ensure that employees had the knowledge and skills required to utilize energy controls, exposing the employees to the hazards of unexpected start-up of machines.
