29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence to achieve energy control by performing the following steps as required by 29 CFR 1910.147(d)(1)-(6): 1.	Turn off and shut down power to the machinery, 2.	Physically locate and operate energy-isolating devices in such a manner as to isolate the machine or equipment from the energy source(s), 3.	Affix lockout/tagout devices to each energy isolating device, 4.	Verify isolation and de-energization of machinery have been accomplished prior to starting the maintenance and/or servicing work:  a) Production Department- On or about December 17, 2021, employees were exposed to machine hazards associated with moving parts while making adjustments on the printing plates with the guards removed and webbing paper when the employee is not in sole control of the operator's control while performing tasks on the printing presses. The employer did not implement energy control application steps as the machine was not shut down or turned off to perform adjustments on equipment [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving dissipation machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limi ted to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR  1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  a) Production Department- On or about December 17, 2021, the employer did not require that each employee working in a team or group performing servicing and maintenance on equipment affix lockout/tagout devices to equipment prior to performing the work, such as adjusting print pads on Printing Press 5 and 9.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) Production Department- On December 17, 2021, the employer did not ensure that all electrical equipment was installed and used in accordance with its listing and labeling. Electrical junction boxes, which are intended to be mounted to a wall and wired with permanent conduit, were not mounted to a wall or fixed location and were used as electrical drop downs to power fans.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  a) Shipping and Receiving Department - On or about December 13, 2021, the employer did not ensure that an evaluation of the operator's knowledge and skill to operate the power industrial vehicles safely was performed every three years for each type of vehicle operated by the employees, including the sit down forklift, clamp forklift, standup forklifts, and walkie/riders.   No abatement certification or documentation is required for this item.
