In every building or structure exits shall be so arranged and maintained as to provide free and unobstructed egress from all parts of the building or structure at all times when it is occupied. No lock or fastening to prevent free escape from the inside of any building shall be installed except in mental, penal, or corrective institutions where supervisory personnel is continually on duty and effective provisions are made to remove occupants in case of fire or other emergency.  a)  On or about 01/04/2022,  at the Culver Manor, undergoing rehabilitation and located at 899 Culver Road in Rochester, New York, the employer did not allow for a quick and safe exit from the building in case of an emergency.  The front door leading to the outside was locked and not marked as "Not an Exit".  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR  1926.150(a)(4):All firefighting equipment shall be periodically inspected and maintained in operating condition. Defective equipment shall be immediately replaced.  a) On or about 01/04/2022,  at the Culver Manor, undergoing rehabilitation and located at 899 Culver Road in Rochester, New York, the employer did not maintain a portable fire extinguisher in good operating condition.  The pressure gauge of a fire extinguisher had been painted over, operators/employees could not tell if the pressure indicator is in the operable range or position.   Employees were exposed to fire hazards associated with inoperable fire fighting equipment.  NO ABATEMENT DOCUMENTATION REQUIRED
