29 CFR  1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  (a) On or about January 10, 2022 and at times prior thereto, the employer failed to develop and implement a written respiratory protection program in accordance with all requirements of 29 CFR 1910.134(c) to protect the employees performing shot-blasting operations in the blast booth from hazardous exposures to respirable particulates in the workplace.

29 CFR  1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  (a) On or about January 10, 2022 and at times prior thereto, the employer allowed employees to wear loose-fitting supplied air hood respirators in the blasting booth area without having a medical evaluation to determine the employee's ability to use a respirator.

29 CFR  1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):  (a) On or about January 10, 2022 and at times prior thereto, the employer had not performed a workplace hazard assessment with written certification for employees working in the blasting booth and painting areas at the worksite.
