29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a) Berryman Transfer and Storage Company, Inc. located at 2000 Moen Ave., Rockdale, IL 60436:  On or about January 24, 2022, and at times thereafter, the employer did not ensure each powered industrial truck operator had successfully completed training and evaluation as required by paragraph (l) prior to permitting the operator to operate a powered industrial truck, thereby, exposing employees and temporary workers to struck-by hazards.
