29 CFR  1910.147(c)(7)(i):The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: (A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the method and means necessary for energy isolation and control.   a) On or about 01/13/2022, general production employees were not adequately trained as authorized employees to recognize the need to lock out the energy isolation device for the Powermatic 2000 Table-saw during the daily maintenance cleaning process and/or during the day when the dust collector clogged with large pieces debris.

29 CFR  1910.147(d): The control of hazardous energy. Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  (1) preparation for shutdown. (2) turn off and shut down of the machine (3) physically locate and operate the energy isolating devices in such a manner as to isolate the machine or equipment from the energy sources.  (4) affix lockout or tagout devices to each energy isolating device (6) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergizing of the machine or equipment have been accomplished.   a) On or about 01/13/2022, during an attempt to unclog the vacuum connector to the Powermatic 2000 Table Saw employees were not shutting off and locking out the energy isolation device by utilizing the specific procedures that requires (d)(1) through (d)(6) to be followed to accomplish lockout.
