29 CFR  1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems:   a) Plating Department - Employees storing and retrieving an aluminum scaffold platform from a mezzanine were not protected from falling approximately 8-feet to the floor below while standing near the open edge, on or about January 31, 2022.  No abatement certification or documentation required.

29 CFR  1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  a) Plating Department - Employees loading and unloading racks were not provided head protection while exposed to a struck-by hazard from steel tubes hoisted and lowered by an overhead crane, on or about January 31, 2022.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identified the workplace evaluated;  the person certifying that the evaluation has been performed;  the date(s) of the hazard assessment;  and which identifies the document as a certification of hazard assessment:   a) Work Floor - The employer had conducted a personal protective equipment (PPE) hazard assessment and provided employees with PPE such as eye, hand, and hearing protection. The hazard assessment did not identify the person certifying that the evaluation had been performed and it did not identify the document as a certification of hazard assessment, on or about January 31, 2022.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
