29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(B) and (c)(4)(ii)(D): (a) DMSS Unloading (R0222 Bag Dump Station) - On or about January 1, 2022, the employer failed to develop procedures to ensure that effective lockout procedures were utilized when employees opened the ports to the rotary vane-valve under this bag dump station exposing employees to an amputation hazard posed by the energized, spinning vane-valve blades.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: (a) DMSS Unloading (R0222 Bag Dump Station) - On or about January 1, 2022, the employer failed to provide adequate training employees exposed to the potential hazards associated with the control of hazardous energy for machines such as, but not limited to, the rotary vane-valve under this bag dump station exposing employees to an amputation hazard posed by the energized, spinning vane-valve blades.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) DMSS Unloading (R0222 Bag Dump Station) - On or about January 30, 2022, the employer failed, the employer failed to protect employees from amputations and other hand injuries by not: 1. apprising an authorized employee of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy before process equipment such as, but not limited to, an energized rotary vane-valve is deenergized; and, 2. ensuring that an orderly shutdown of machine process equipment such as, but not limited to, an energized rotary vane-valve, is utilized to avoid any additional or increased hazards to employees as a result of equipment stoppage.
