29 CFR  1910.22(c): The employer did not provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces.  a) Warehouse, Extrusion Department: On or about February 9, 2022, and at times prior, the employer exposed employees to trip  hazards, in that the employer did not maintain access free of debris and obstructions where employees access the back stairs of the roller lines to move between walking-working surface levels in the performance of assigned job duties.

29 CFR  1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  a) Warehouse, Extrusion Department: On or about February 9, 2022, and at times prior, the employer exposed employees to respiratory hazards, in that the information contained in Appendix D to 29 CFR 1910.134 was not provided to the employees who wear N-95 filtering facepiece respirators on a voluntary basis in the performance of their job duties.

29 CFR  1910.1200(h)(1):Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.  a) Warehouse, Extrusion Department:  On or about February 9, 2022, and at times prior, the employer exposed employees to  chemical hazards, in that  effective information and training on hazardous chemicals including, but not limited to, limestone and crystalline silica were not provided to employees while performing duties associated with production of tiles.
