29 CFR 1910.119(e)(3)(i):The process hazard analysis did not address the hazards of the process: On or about February 21, 2022, in Hydrocracker unit #215 at Marathon Petroleum refinery located in Garyville, LA, workers were exposed to fire and explosion hazards when the employer did not identify, evaluate and control the hazardous condition of flammable recycle gas and/or hydrogen gas potentially flowing into the startup ejector #215-2801 and associated piping from the recycle gas compressor suction Knockout drum #215-1207.

29 CFR 1910.119(f)(1)(i)(G): Employer did not develop and implement written operating procedures that provide clear instructions for safely conducting Startup Procedure following a turnaround: a) On or about February 21, 2022, in Hydrocracker unit #215 at Marathon Petroleum refinery located in Garyville, LA, workers were exposed to fire and explosion hazards when the employer did not ensure that the steps to operate 6-inch inlet valves for the ejector #215-2801 were written in the startup procedure for Hydrocracker Unit #215 and it was performed accordingly as part of its return to service process.
