One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:  a) On or about February 24, 2022, at a workplace located at 100 Industrial Drive, Sheridan, WI, in the Printing Department: an unguarded belt and end roller created an in-going nip point that was positioned 26.5 inches from the floor, on the M132 Printing Press Line on the outfeed conveyor preceding the folder, exposed employees to a potential pinch point hazard.   b) On or about February 24, 2022, at a workplace located at 100 Industrial Drive, Sheridan, WI, in the Printing Department: eight unguarded belt and pulleys on the Stacker Unit on the M132 Printing Press Line, created in-going nip points that were positioned 26 inches from the floor, exposed employees to potential pinch point hazards.   c)  On or about February 24, 2022, at a workplace located at 100 Industrial Drive, Sheridan, WI, in the Binding Department: three unguarded belts and pulleys at the corner of the transitioning machine created in-going nip points that were positioned 40.5 inches from the floor, on the 850-6 Stitcher Line.   d) On or about February 24, 2022, at a workplace located at 100 Industrial Drive, Sheridan, WI, in the Printing Department: three openings, approximately 12 x 6 inches, above the bottom chill rollers on the MB Printing Press, exposed employees to potential caught-in hazards. The openings were 33 inches from the floor.   e) On or about February 25, 2022, at a workplace located at 100 Industrial Drive, Sheridan, WI, in the Binding Department: unguarded belt and pulleys on each of the stream feeder units (1-16) on the Prim binder machine exposed employees to potential pinch point hazards. The pinch point hazards were 33.5 inches from the floor.   f) On or about February 25, 2022, at a workplace located at 100 Industrial Drive, Sheridan, WI, in the Binding Department: four unguarded belts and the end roller on the Muller binder stacker exposed employees to potential pinch point hazards. The belts were 36 inches from the floor.   g) On or about February 25, 2022, at a workplace located at 100 Industrial Drive, Sheridan, WI, in the Binding Department: the manufacturer supplied cover was missing on the backside of the Muller binder equipment exposing a rotating belt and end pulleys.   h) On or about February 25, 2022, at a workplace located at 100 Industrial Drive, Sheridan, WI, in the Binding Department: two unguarded belts and pulleys on the 850-7 Harris Graphics card feeder 2 created an in-going nip points that were 31.5 inches from the floor, exposed employees to potential pinch point hazards.   i) On or about February 25, 2022, at a workplace located at 100 Industrial Drive, Sheridan, WI, in the Binding Department: two unguarded belts and pulleys on the 850-7 Harris Graphics card feeder 4 created an in-going nip points that were 31.5 inches from the floor, exposed employees to potential pinch point hazards.   j) On or about February 25, 2022, at a workplace located at 100 Industrial Drive, Sheridan, WI, in the Binding Department: two unguarded belts and end rollers on the 850-7 stacker created an in-going nip points that were 38.5 inches from the floor, exposed employees to potential pinch point hazards.
