29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:  a) Worksite, 231 Upper Mountain Ave, Montclair, NJ  The employer did not protect employees working in an excavation to connect a 10 inch PVC pipe at the bottom of a trench that was approximately 5 feet 5 inches deep and 3 feet wide in an area approximately 5 feet in length, from the hazard of cave-ins or collapse.   Violation occurred on or about March 3, 2022.
