29 CFR 1904.7(b)(3):  When an injury or illness involved one or more days away from work, the employer did not record the injury or illness on the OSHA 300 Log with a check mark in the space for cases involving days away and did not entry of the actual or estimated number of calendar days away from work in the number of days column:  a.)  1863 Bethany Road, Womelsdorf, PA 19567 - on or about March 18, 2022, the employer did not classify Case No. 1 on the 2022 OSHA 300 log as a "days away from work" in column H for an incident that occurred on March 11, 2022.  a.)  1863 Bethany Road, Womelsdorf, PA 19567 - on or about March 18, 2022, the employer did not record the actual or estimated number of "days away from work" for Case No. 1 on the 2022 OSHA 300 log for an incident that occurred on March 11, 2022.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1904.29(b)(1):  A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation:  a.)  1863 Bethany Road, Womelsdorf, PA 19567 - on or about March 18, 2022, column F for case no. 1 on the 2022 OSHA 300 log did not provide a description of the injury.  b.)  1863 Bethany Road, Womelsdorf, PA 19567 - on or about March 18, 2022, column F for case no. 1 on the 2022 OSHA 300 log did not accurately describe the part of the body affected.  c.)  1863 Bethany Road, Womelsdorf, PA 19567 - on or about March 18, 2022, column F for case no. 1 on the 2022 OSHA 300 log did not accurately describe the object that directly injured the person.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
