29 CFR  1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall shall be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level;  a. Located at 3288 Hairst Drive, Reynoldsburg, Ohio: On or about March 15, 2022, employees were roofing a new-build two story home while using personal fall arrest harnesses attached to rope lifelines. The employer did not ensure the lifelines were rigged to prevent a free fall of less than 6 feet, thereby exposing employees to a fall hazard.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  a. Located at 3288 Hairst Drive, Reynoldsburg, Ohio: On or about March 15, 2022, an employee was roofing a new-build two story home while not attached to a lifeline, thereby exposing the employee to a fall hazard from approximately twenty feet.  Zetico Martinez was previously cited for a serious violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1479745, citation number 1, item number 1b and was affirmed as a final order on 08/18/2020, with respect to a workplace located at 935 Burtwood Ct. Pataskala, Ohio 43062

29 CFR  1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached.  a. Located at 3288 Hairst Drive, Reynoldsburg, Ohio: On or about March 15, 2022, employees were roofing a new-build two story home while using personal fall arrest harnesses and ropes that were tied around the peak of roof trusses and nail plates. The lack of proper anchorage thereby exposed employees to a fall hazard from approximately twenty feet.  Zetico Martinez was previously cited for a serious violation of this Occupational Safety and Health standard 29 CFR 1926.502(d)(15) which was contained in OSHA inspection number 1479745, citation number 1, item number 1c and was affirmed as a final order on 08/18/2020, with respect to a workplace located at 935 Burtwood Ct. Pataskala, Ohio 43062.
