29 CFR 1910.22(a)(3): (3) Walking-working surfaces are not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, holes, tripping hazards, spills, snow, and ice.  a) 640 Dell Rd Carlstadt NJ Flatbed truck: Employees were exposed to tripping hazards while working on the flatbed of a  truck floor surface. The flatbed of the truck had holes, loose metal plates and broking weld, on or about 3/16/22.  b) 640 Dell Rd Carlstadt NJ loading dock: Employees were exposed to a tripping hazard while transitioning from the flatbed truck to the loading dock of the warehouse. The loading dock was not flush with the trucks bed, on or about 3/16/22  c) 640 Dell Rd Carlstadt NJ loading dock: Employees were exposed to a tripping hazard while transitioning from the  loading dock of the warehouse on to the truck. The loading dock was not flush with the truck bed creating tripping hazards, on or about 6/13/22.
