29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee, who wore prescription lenses while engaged in operations that involved eye hazards, used protection that incorporated the prescription in its design, or wore eye protection over the prescription glasses or protective lenses with side shields:  a) On March 18, 2022 and times prior employees wore prescription glasses, without side shields, while working with flammable liquids and powders where the safety data sheets call for safety glasses with side shields or goggles.  These materials include but may not limited to, Cab-O-Sil and Xiameter.    Note) Certification of corrective action is required for this item.

29 CFR  1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects:  a) Compounding area: On March 18, 2022 and times prior employees used a Waring handheld immersion blender to smooth Cab-O-Sil (untreated fumed silica) into a batch.  The plug on the electric blender was missing the grounding pin.   Note) Certification of corrective action is required for this item.

29 CFR  1910.334(d):  Where flammable materials are present only occasionally, the employer did not take measures to prevent hazardous conditions from developing  when electric equipment capable of igniting them was used.  Such materials include, but are not limited to: flammable gases, vapors, or liquids; combustible dust; and ignitable fibers or flyings:  a) Compounding area: On March 18, 2022 and times prior employees were exposed to a flash fire hazard when using a Waring Immersion Blender to smooth Cab-O-Sil (untreated fumed silica) into a batch.  All metal parts of the mixing equipment were not properly earthed and/or grounded.    b) Compounding area: On March 18, 2022 and times prior employees were exposed to a flash fire hazard when using a Waring Immersion Blender smooth Cab-O-Sil (untreated fumed silica) into a batch.  The E-Z Arm Extraction System was not near enough to the mixing kettle to remove buildup or airborne dust particles.   Note) Instance a - Corrected during inspection.
