29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a) On or about 03/14/2022, and at times prior to - at 7595 Gadsden Hwy, Trussville, AL 35173, Department 27, Box Machine Pack-Out -employees operating the Box Machine were exposed to moving machine parts which could cause contusions, fractures or disfigurement including amputations because the guard did not adequately prevent access to moving machine parts while the machine was operating.
