29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about March 31, 2022, the employer failed to develop, document, and utilize machine specific energy control procedures for service and maintenance activities such as, but not limited to inspection, repair, and replacement of filters performed by authorized maintenance employees on baghouses #1, #2, #3, and #4. These employees entering the baghouses are potentially exposed to hazards such as, but not limited to disorientation within the baghouse due to atmospheric conditions; slips, trips, falls; being struck-by objects and/or hazardous atmospheres upon unexpected energization of electrical energy sources due to the lockout device not being under the exclusive control of the authorized employee performing the service or maintenance.
