29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)This violation occurred on or about March 31, 2022, at the job site located at 11862 CR 140, Flint, TX 75762; where employees were applying flashing to the roof of a new residential two-story single-family house and were exposed to head injuries where overhead hazard exists by not wearing hard hats.

29 CFR  1926.102(a)(1):Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  (a)This violation occurred on or about March 31, 2022, at the job site located at 11862 CR 140, Flint, TX 75762; where employees were applying flashing to the roof of a new residential two-story single-family house while using a drill, model number and serial number could not be recognized. Employees were not wearing safety glasses and were exposed to eye injuries.

29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  (a)	This violation occurred on or about March 31, 2022, at a job site located at 11862 CR 140, Flint, TX, where employees were doing roof flashing work while working from a wood man made basket attached to a Genie lift. Employees did not have on a body belt or safety harness and lanyard and was exposed to a fall hazard.
