29 CFR 1915.56(b)(2):Welding cable was not free from repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected. On or about 04/05/2022, employees were exposed to an electrical hazard at 1149 East Fifth Street, Ashtabula, Ohio 44004; the employer did not ensure the employees repairing the CALUMET, while it was docked, were protected from a cable with a splice within 32 inches from the cable end to which the electrode holder is connected and used exposing employees to electrical shock hazards.

29 CFR 1915.56(b)(4):When welding cable became worn to the extent of exposing bare conductors, the portion exposed was not protected by means of rubber tape, friction tape, or other equivalent insulation. On or about 04/05/2022, employees were exposed to an electrical hazard at 1149 East Fifth Street, Ashtabula, Ohio 44004; the employer did not ensure the employees repairing the CALUMET, while it was docked, were protected from welding cables that were worn to the extent of exposing bare conductors exposing the employees to electrical shock hazards.
