29 CFR  1910.119(f)(3): The operating procedures were not reviewed as often as necessary to assure that they reflect current operating practice, including changes that result from changes in process chemicals, technology, and equipment, and changes to facilities.  a) On or about April 14, 2022, in the Blending Area; VAT A operating procedures did not address the potential hazard of ignition sources such as, but not limited to static electricity when adding flammable materials to VAT-A during blending operations, exposing employees to fire and explosion hazards.
