29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. Location - 221 Concord Avenue, Belmont MA. On or about April 20, 2022, an employee was standing on the ground as a truck lift gate was lowering a cart with pipe load on it. The cart rolled and tipped over crushing employee.
