29 CFR  1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  (a) On and before April 25, 2022, the employer did not ensure that the workplace was assessed to determine if hazards were present which would necessitate the use of personal protective equipment.  Employees operate hot glue rollers, press machines, and vertical band saws. This condition exposed employees to hazards associated with these tools and equipment.

This condition exposed employees to burns.

29 CFR  1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  (a) On and before April 25, 2022, the employer did not request that a Nissan, Model Number: CUM01L18S Forklift be marked to identify the MECO Omaha FORKLIFT STOCK PICKER attachment and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. This condition exposed employees to fall hazards greater than 10 feet.
